i believe that they are referring to genetics, but it could be the other way around if the kid was ugly when he/she was born and the parents were plastic surgeons they could create the perfect child of there dreams, but then when the kid grew up and had a baby it wouldnt look at all like the mother, and then the father would learn that his wife used to look like $hit.

most of the time genetics
if the parents and child both have had the same plastic surgery done then it probably a joke about that.
