Doctors say that most toilet germs only go about 2 feet from the toilet once you flush and then die out. So you best bet would be to keep your toothbrush as far away from your toilet as possible. If for some reason your bathroom is really tiny (im talking only 2 feet wide) or you are really paranoid then you can by a toothbrush cover.
However, most of the germs die anyway so you have nothing to worry about.

The only trouble with covering a toothbrush is they don't get a chance to dry out, which is just as bad, in some cases they get moldy, so I wouldn't worry so much about the urine splashing, I would just rinse the brush in hot water before use, although they do say that when you have a cold or the flu, that it is important that you throw away the brush and get a new one.
A nice thing about some of the newer houses, is that the commode is built a little bit away from the tub and sink.

It's been shown that urine splashes many feet. It's also been shown that it doesn't seem to matter where you keep your toothbrush in the bathroom, it will still have some minute amount of fecal matter on it. If I were you, I'd say 6 feet from the toilet, or not in the bathroom at all! When you flush the toilet, Nasty fills the room.

I don't know about urine splashing, but I heard that toothbrushes should be 6 feet away from the toilet because of germs released into the air when you flush the toilet. Kind of scary, huh?

I've heard that you should keep your tooth brushes in the medicine cabinet, because every time you flush bacteria from the toilet gets thrown into the air....or you can also just close the lid before you flush.
