It is a greeting and the usual answer is "fine, thanks, and you?" or as like they say in Spanish "como esta?" and the other person replies "muy bien, e tu?" It is just a greeting, unfortunately there are some people out there that think when you greet them this way, you are asking for a run down on their health, life etc. then it is time to say, "oh my goodness, look at the time! I will have to catch up with you later!", and then remember never greet that person like that again!

Usually it's a greeting. No body really wants to hear your good/bad news when they first see or meet you. It's just the proper well mannered thing to say at occasion.
