start slow at first 3 times a week, especially if this is your first time in the gym..

start with a light warmup on the treadmill for approx. 10 minutes, then do your weights for 25 minutes, concentrating on overall body muscle development (chest, shoulders, back, legs, arms, and abs) one exercise for now per body part, 2 sets of 10 reps.

then do some cardio, as long as you can up till 30 minutes, if you can only do 10 minutes then great. eventually you will be able to do more. patience is key, to seeing results.

as for a diet, a good 4 to 5 meals a day is sufficient concentratin in foods that are high in protein, carbs, and fats (the good stuff) and lots of water make sure you are drinking at least 2.5 litres per day.

I'm hesitant to answer this question as I'm not terribly familiar with diet and exercise for guys your age, but I'll answer anyway.

As long as you don't feel any pain and your doctor says you are ready for anything, then you are ready for anything.

Start with just 30 minutes of exercise 3 days a week and work your way up. As for diet, that's really up to you since there are so many options available, as long as you know the obvious- which is that there is no magic pill and starvation doesn't work.
