Recipes are correct - just whiskey and sweet & sour mix. The original recipe was for the drink to be blended and strained, but these days most people order it on the rocks - use a double old fashioned glass, packed w/ ice.
And, these days, more people order Amaretto sours as opposed to whiskey sours. Same procedure, just substitute amaretto for the whiskey.
The usual garnish is a marachino cherry - never heard of using a mushroom, sprout, etc.

I've made it with whiskey, sour mix, powered sugar, marischino cherry syrup, and garnish with a cherry (or four, if you're into cherries). I'm sorry that I forget the amounts off the top of my head! I make mine extra-sweet though. I love the taste of sour mix but adding the sweetness to it makes it oh so tasty. Anyhow, shake all that up in a shaker with ice, then strain it into a drinking glass that's filled with ice, and make sure you have straws on hand because I think whiskey sours taste so much better with a straw! *grin*

whiskey and sour soda, and then the condiments (mushrooms, olives, sprouts etc.). if you add a few drops of bitters and crush a sugar cube in there, you will have a whiskey old fashion sour.
