I would wrap a box and it's cover in very pretty wrapping paper and bows. Make it so the box can be opened without tearing the paper.
Then I would wrap up little gifts and put them in there. I would also enclose things that would remind her of home like a book of poems with a flower from the garden pressed between the pages. Maybe wrap up her favorite coffee mug. Or a bag of her favorite candy (nothing that will melt or get sticky). Maybe a small photo album with copies of some of the family pictures in it. A lock of your hair with a ribbon around it. Some silly stuff too like a coloring book and crayons or a jump rope. Maybe a CD you knew that she wanted and didn't get... Or write her a letter and wrap it telling her all the ways that you love and miss her.
Put them all in the box and then write a poem to put on top of the box saying something to the effect that:
When you are gone and we are apart,
Pick a gift from the box to heal your heart.

A great photo of you two together in a Best Buds or Sisters frame and a heartfelt card telling her how much you love her and are going to miss her.

A journal so she can write of her adventures and share them with you when the two of you reunite is a wonderful gift.

A prepaid calling card with a note saying that she can use it to call anyone, but you'd love it if she called you, too, once in a while. If she's young it's not likely that she'll be able to afford long distance calls.
