Usually yes. You are supposed to only touch the table with your hands - possibly with your forearms on the edge, while not eating. Otherwise it looks sloppy...

Yes it is - elbows should be removed from the table at all times
ignore McDonalds and Taco advertisements as they do not accurately reflect the proper standards of english nor french etiquette.
Also - make sure your knife and fork is on the plate in correct fashion when chewing. Mouth closed at all times between bites and spoon placed inside the knife on the righthand side when setting the table. the fork(s) are on the left flanked by the napkin and wine glass always on the right-hand side on an angle above the knife

I've read that it's because it looks like you are bored when you put your elbows on the table. I think differently, from experience. My sister spreads her elbows out on the table, taking up a lot of space.

Years in the past a female could take a seat on the table and then take off her gloves. A gentleman could take off his gloves and then take a seat to dine.this is been going downhill ever because that prepare went out of style.
