Cancer has always been around, and was mentioned by Hypocates more than a couple of thousand years ago, so there probably is no real answer to that question except Hypocrates.

It depends on the patient and the type of cancer - a Hematologist or Pathologist would diagnose cancers of the blood and bone marrow, and a Pathologist would diagnose cancers from biopsy specimens. The patient is then often referred to a specialist that treats that particular type of cancer or to an Oncologist.

No idea but The Germans were the first to link cigarette-smoking and lung-cancer pre WW2.
