It's a chance for the doctor to see that you're growing and developing normally. It's also a chance for your parents to talk with the doctor about any issues, such as safety or nutrition, so they can help you stay healthy. And it gives you a chance to ask any questions you might have about your health. You might wonder when you will grow taller, for instance, or if you weigh the right amount.
The nurse might start by checking your weight and height, temperature, blood pressure and maybe collect a urine sample.
The doctor will come in and say hello. He or she will probably ask you some questions, like how you're doing and if you have any problems or concerns. The doctor wants to make sure your body is working just like it should. To do this, he or she will use equipment, such as a stethoscope (to listen to your lungs and heart), an otoscope (to look inside your ears, nose, and throat), and an ophthalmoscope (to look inside your eyes).
Your doctor might check the reflexes in your knee with a rubber hammer. He or she also will probably feel around your belly, and examine your genitalia if necessary.(they'll be others there so don't feel uncomfortable), also examine your spine.
Don't worry, it's just a check-up to make sure that all is well with you.

You mean for a boy or a girl? When I was 12, my doctor examined me, checked my growth, my weight, to see if I was still growing (I was), to see if I had started my cycle (which I did) and just asked me questions like did it hurt when I peed? or (for a boy) do you have any type of dreams you would like to tell me about? What type of physical activities was I doing...things like that.

Everything the other people have said, and --
the doctor will listen to your heart in four places on your chest, and four places on your back.
the doctor will listen to you breathe, with his stethoscope on your chest, and on your back.
He (or she) will tap you on the knees with a rubber mallet to test your reflexes.
He will look into your mouth, shining a flashlight.
He has a specialized flashlight for looking at your eardrums.
He will ask you about what you've been eating and whether you've been exercising.
The nurse will take your blood pressure using a cuff on your arm.
The nurse will measure how tall you are, how much you weigh, and take your temperature.

The doctor checks temperature, blood pressure, height, weight, inside your mouth. Also, if you are male he will also check your testicles and groin area for any abnormalities such as cancer. Sometimes they will have you lean over and check your anal area. Sorry, but that's what they do in pediatrics.

Honestly-- not much. they measure you-- make sure you are growing. Look at your ears, eyes, nose. Examine your back for curvature of the spine. Ask you if you have begun menses.
Ask you about vitamins and minerals
