It is a mix. A lot of is the parents fault for leaving it all for everyone else to do. The teachers and administrators can only do some much because of all of the red tape the politicians are making them do. You spend most of your time filling out papers instead of teaching. Then you have teachers who don't care because of all the stuff they have to do.

1.Standardized testing from politicians (high scores= higher tax rates for homeowners.)
2.Teachers are underpaid and overworked. (Administration/politicians)
3.More parent support is always helpful.
4.Teachers who get into education for the wrong reasons.
Good question, but I'm not sure if there's an easy answer. These are MY opinions.

I think it's mostly a political issue; the government has its priorities screwed up. The focus currently is on tax relief for the rich; educational programs are constantly being cut.

It's also on a local level, where cities don't vote for educational funding bonds.

Teachers are doing the best with what they have.

The ones listed in your question, changing demographics, attitudes toward education and "experiments" in education just to name a few. Parents---education begins in the home. If the child is struggling in school the parent need to talk with the teacher to see what needs to be done. Not blame a beuracrat for not giving their school enough money.

Ditto...all of the above, but even more so, the parents. It is ultimately in their power to push for a better system and also their duty to actually instill responsiblities into their children.
