SLOW DRIVER ON THE FAST LANE
AND PEOPLE THAT DON'T MOVE OUT OF THE WAY WHEN THEY KNOW THEY ARE BLOCKING OTHERS

Driving too slowly in the fast lane, slowing down to ENTER the freeway, getting in my way and generally driving like an idiot.

in order of decreasing frustration:
slow in the fast lane (especially when there is room in the slow lane and they aren't passing)
using brakes on highway just to slow 2 mph
tailgating
slow/careless when talking on the phone
