Some are, just as some people are..because they don't eat right and exercise enough...or can't metabolize right..

You can tell if a dog is fat by feeling his/her sides. If you can't feel the ribs, the "dog fat"

Some can be, due to a few reasons.
You should be able to feel a dog's ribs when running your hand over them, but not see them.

Yeha sometimes, when they eat too much. Or sometimes, puppies are born like that.
