I have all of those things, and I'm highly functional in most cases. The thing with anxiety/shyness/agoraphobia is to take small steps. Can't walk out the door? Try holding the doorknob for awhile, until you feel comfortable enough to go to the next step, which might be opening the door, standing there for 20 seconds, and then close the door. Either way, it's small steps.
As for shyness? Try getting together with one friend for one day a week. Do something at home like watch a movie, cook something, play a board/card game, etc. As you gradually become more comfortable, have 2 friends over, then 3, etc. Until you can host a party of 5 friends or so. Then try going to a small public place--a park, a small cafe, whatever. It's all BABY STEPS.
What also helps is making a "hierarchy", a list of things that make you increasingly uncomfortable, on a scale of one to ten. Ten would be your ultimate worse nightmare; one would be something you could do without any anxiety. Build it up. Think of something simple--would going out to dinner by yourself be on the lower or upper part of your scale? After making the hierarchy, practice some of the lower things until you can move up to the next level.

Shyness and agoraphobia could both be linked to a social anxiety disorder. I suggest you talk to a medical professional about this condition, and see what they recommend. Anxiety can be controlled by the use of drugs such as Lexapro, Paxil, and others. Living with anxiety is not fun. Talk to a doctor.

Being shy is not a disease it is perfectly normal and it is something that you can overcome. I believe it also stems from concerning yourself about what other people think of you.
Focus on the positive things and not the negative because if you contiually focus on the negative you will always be worried about something.
I would highly recommend reading the book, "The Magic of Thinking Big" by David J. Schwartz. It is a very good book and it is also a fun read.
