to a urologist first,
If he has no issues, then disciplines are next: take away things that he likes. insure that he does things that he does not like.
if he wants to cry, let him but do not stop what it is you are doing.

My oldest son is 61/2 years old and has been potty trained since he was 2 years old. They might be problems in with how the parents are handling this, simply for the fact that he is still having issues with this. I just finished potty training my seond son couple months back. It took him five days to be potty trained to pee in the toilet. For that we did NOT use training diapers, those dont really helped. What really helped was wearing real underwear, and he would pee on himself at first, and he didnt like that at all. The pooping took about a month, we used a sticker chart and taking away his animals one by one everytime he pooped in his underwear [He LOVED his animals], and everytime he pooped in the toilet, he could chose which animal he wanted back. The chart and getting the animals pumped him up to go in the toilet.
But from the sounds of it, he is most likely having this problem because of the parents, or whoever is potty training him. They need to stop being so lax about it and try a different way of helping him. He is much too old to still not be potty trained

My kid's are grown now but I never had problem with potty training them. I used to sit the potty seat in front of the t.v. and they would sit there til they went. After they would go I would alway's make a big deal about it and reward them. If your nephew is 6, there need's to be some serious training. Just get some big boy undergarments and make him where them and when they get wet he won't like that feeling too much he'll want to use the potty. Good luck!!

go every 15 mins let them pick out cool under garments like barbie or spiderman whatever they like. then dont put a diaper on them .A step stool with their name painted on it is special and they can climp up to the big potty and they have seats that fit on there and some even glow in the dark. you can use stickers or stars on a calendar.

Give him the reward of having big boy underwear on. & ask him regularly if he has to go potty. & if he has an accident explain to him that if he has to go to the bathroom he needs to use the toilet. & let him pee outside sometime...most boys like that.

attack the cheerios..... put cheerios in the toilet and tell him it's a game.... tell him he has to try and sink the cheerios... if it's a bowel thing.... I think just make sure that he has a regular schedule of going to the bathroom... for my son we set a timer that went off every hour... when that timer went off that's when he had to go...eventually he had to do the number 2... and then the timer eventually went away... till the next one needs it..

Take him to a urologist and have him checked out. He may have developmental problems also.
