People who don't know reality from fantasy are often a danger to themselves or others. They are kept in the hospital (ideally) for safety and to help them get better if at all possible.

I would say that it depends on the situation. If, say, the person's inability to understand reality makes him or her physically dangerous to self or others, then it would make sense to seek containment to prevent harm (the imprison part) and therapy to address the lack of understanding of reality.
For example, a kid who makes animal sacrafices because he or she must carry out the orders of some god in some video game probably should not be allowed to continue doing what he or she is doing. If the belief persists, there should definitely be some mental help sought out for the kid.

People like this are not usually able to take care of themselves, so they have special needs that only a trained professional can provide, which makes a group or assisted living community an option, or a looney bin. But if they don't understand reality, then do you really think they feel imprisioned in the first place?

It is not a question of fairness, but a question of what is in the best interest of the person and society. A person that does not understand reality may commit a crime because there reality tells them that their action is ok. Also their reality may tell them that they can do things to themself and not be harmed by it.
This person could unknowingly hurt themself or others.

I do not see this as an imprisonment - if they have a break with
reality they are not functioning on a mental level. they are out
of touch and need help. They can hurt themselves and others
if they are not careful.

You wouldn't want that would you?

If they do not understand reality it will not matter where they are, because they will not understand if it is real or not!

Yes, because they could be a danger to themselves and others. I think it sad, but it is what's best for them and others.
