Go to your family doctor... they will give you a test and a questionnaire for a friend or loved one to fill out...... it's called a Conner's test.... it ranks your ADD and helps the doctors give you the right dose of medication.

You can just make a regular appointment with any family practice physican. Below I have pasted the test that they will run. They run similar tests ADD & ADHD. Best of luck to you~ Take Care :)
An interview with the child.
Medical history, to include asking a parent about the social, emotional, educational, and behavioral history of the child.
Physical examination.
Behavior rating scales or checklists for ADD given to parents and teachers to evaluate the child's symptoms.

you can be diagnosed with add pretty much by any psychiatrist you see. that stuff is overdiagnosed nowadays. but i must say the medicine is great! lol when you need it of course.
They will probably give you papers to give to your teachers (if youre still in K-12 schooling)asking them to fill out some fact about your behavior etc...
i would recommend you to only get diagnosed by this if you REALLY need it, i dont care what the doctors say, but it chages personalities in a way, its like youre not yourself.
if you can control yourself than do so. but i guess you can get some pills for when you really need to focus and work, know what i mean? and not just the daily dose one is supposed to take when diagnosed.
