The top two answers are correct. The IRS can garnish wages, but so can creditors - however, depending on the state they may or may not need a court order, liens can be placed on your homes, savings accounts show on credit bureaus.
Companies that collect for medical bills will be very aggressive with collecting these debts. Get help if needed.

Yes, depending on how far the collectors are willing to go. You should at least make an attempt to pay. Didn't you get the help you needed?????
