i need to put an adobe file into microsoft word?

ok well you see i need to take an application for tim hortons which is an adobe file as you can see here http://www.timhortons.com/en/pdfs/tth-application-us-en.pdf

but i wanted to fill it out on the computer to make it look more professional will somone please tell me how to take this file and make it into a word document so i can add text boxes to fill it out?
