Failure Modes and Effect Analysis (FMEA) for design is used to analyze the reliability of a particular product during its development phase. After listing down the failure modes foreseen and their effects, the analyst ranks them according to their severity, occurrence and detection (a scale of 1-10 is employed, 10 being the highest). These ranks are used to compute for another parameter (I've forgotten what it's called) and this is what you use to determine which failure mode is the most critical.
