(you probably could get away with "but I forgot exactly what it was")

The 2nd one, "I knew I had to remember something but I forgot what that exactly was".
When using past tense, you have to change "it" with ""that".

The third is the only correct one, but unless you are writing this for an English paper or something, you could use any of them and be fine. The first two are only wrong because they end with a preposition (i.e. 'was', 'at', etc.), but 'fluent' speakers use these anyway, especially in southern America.

The third one is correct. You could get away with the first one, but the third sounds better.
Actually you could get away with any of the three, since most people that supposedly speak English 'fluently' don't actually understand proper grammer! It's quite annoying.
