You are saying "Hello, is there anyone there, will you talk to me, are you the person I was trying to contact." Convention simplifies it to just one word "Hello?" Everyone else has an idea of what you are saying and asking so they know how to respond.
Also "Hello" is a formality to open a conversation. It tells the other person that you are there and want to talk with them. Many people ignore this and other polite conversation bits, and that's wrong. Politeness is the grease that keeps societies wheels running.

It depends on your purpose. Some people just say hello.
"Hello. I am Jane Jones and I am Jim's friend."
Others say hello as part of a sentence with the rest unspoken.
"Hello?..." (is there someone there?)
Still others say hello and ask the question with it.
"Hello?, May I speak to Betsy?"

When the telephony exchange was 1st created people used to answer with AHOY. As phones expanded and people started getting calls from people that they did not expect they would answer AHOY? Ahoy was later replaced as a greeting with Hello. But the same question format was carried over to it as in HELLO? "Who should I say is calling?"

You're greeting the person & trying to figure out who it is at the same time. So it won't be rude. You wouldn't the phone & say, "Who's this?!" When it could be your grandma. It's the sounds more nicer when you say, "Hello?".

You're saying "Hello?" as in asking a question because "Hello" in this context means "Is anyone there?" This is probably because people usually said "Hello" or "Blahblah speaking, who's this?" and they sort of merged together into "Hello?"
Imagine if it was "Blahblah speaking, hello?"

It is to see if anyone is there. Sometimes people hang up. It's also a nice greeting.
