should i add "to" when i use "help" in a sentence? i mean sometimes i hear people say "..... help me to (a verb)...." but sometimes i also hear people say "...... help me (a verb)..."? which one is right? thanks.

I think you have confused the 'GEREND' with the 'INFINITIVE'.
Gerend is a verbal noun.
Explanation:
To take = taking
To play = playing
To make = making.

Here, To take, To play and To make are infinitive. Here you have to use the verb in simple presentense after 'To'. If you want to use the 'gerend' in place of 'infinitve', you need not use the word, 'TO' but the verbal noun has to be used like taking, playing and making. See that the verbs are ending with -ing.

eg. I help you to pick the ball (correct). I help you picking the ball (correct). Both the sentences are correct. Here, 'To pick' is equal to 'Picking'. So, the usage of the word 'TO' depends on you, whether you are using gerend or infinitive.

the other answers seem to have answered your question, but I thought I would say 'how wonderful it is to see someone who actually cares about grammar'. Congratulations. I am known as the spelling/grammar nazi at my workplace. Some people say to me 'it doesn't matter'. It matters very much, as we are losing our writing skills it seems, along with grammar and spelling.

Using the word "to" is unnecessary. It can be omitted.

Both 'help (somebody) do something' and 'help (somebody) to do something' are grammatically correct.
