You MUST get that from your employer. They MUST provide it for you. You CANNOT "reconstitute" a W-2 either yourself or with the help of a tax professional.

Dear Wost-2 Form,

I usually write long and lengthy letters but the answer to this is really simple... Just contact the employer that you worked for and they should be able to provide those forms to you. Most employers retain this information for at least 7 years.

If your employer can not produce the W2 Forms, then either try and find all of your paystubs or the last paystub of the year. It should have your total deductions and monies earned on it. You can then take your last paystub with you to a CPA or tax preparation company and they will "create" a W2 Form for you based off that pay stub or the collection of all of your stubs.

You'll have to pay an extra fee for this but it will be worth it if you can get a return on your filing.

Contact your employer and the IRS. Hopefully between the two, the matter can be resolved without much problem.
