Hi,
First - try thinking and searching where and how it got lost or misplaced
second - if the first doesn't work, then try asking the insurance agent you bought the policy from.
Third - if the first and second are both futile, then you can go to the insurance's branch office (any branch) and request for a copy. Pls provide the relevant documents, such as an identity card or maybe the policy holder's social security number.
Fourth - if the branch office says that they can't give you a duplicate of your policy, then maybe you should head to the HQ.
Fifth - if they still are not giving you your duplicate policy, make noise, threaten to send letters to the media, etc.
Sixth - try not to lose the duplicate.
Bye!

Are you trying to collect a claim? Has the policy matured?
If not, then you will probably get a notice in the mail that a premium payment is due, and you will have the info you need on the statement.

Contact the company -- even online. They will need a statement saying it is lost or destroyed -- but getting a new policy is very easy.

You can check the person's bank statements to see which company insurance payments were sent to.
