The smallest state is Rhode Island.
The smallest county in Rhode Island is Bristol County.
The smallest town in Bristol appears to be Warren, but I cannot verify that (population 11,500).

Incidentally, to be a 100% accurate answer you would need to define the term smallest. I went with geographical area for state and county, then population for town. I found two other towns named in the county but the census bureau doesn't track their population separately.

And also incidentally, the smallest town does not have to necessarily be in the smallest county or the smallest state. For example, King County Texas has an estimated population of only 323 people for the whole county. I did not mention it in the original answer because you asked for the smallest in the smallest in the smallest and Texas would never come into consideration as the smallest state.
