We booked as a group (2 couples) and don't want to go without the other couple. This was going to be a once in a lifetime holiday. We both have annual travel insurance policies. What are the chances of us getting our money back and what is the best way to approach the insurance company?

Alternative possibility. See if they'll reschedule. Surprisingly, many companies will work with you. They don't want you to start a negative advertising campaign against them.

Or contact your friends doctor, and see if he'll write you a note too.

Depends on what your policy says - read it all the way through. Some allow cancellation because travelling companion is ill, for others they only pay if it's the actual traveller or, usually, a close relative.

But it never hurts to ask them anyway.
