It reads

will____attend

If I want to attend do I leave it blank or check it or write "want to", "love to", "like to". If I don't want to attend do I write "not"?

Writing the number of people sounds strange.

Will 2 attend

Is that proper grammar?

Perhaps I can write in 'we' in front of 'will' and write 'both' in the blank.

Thats an odd rsvp card.....I would put down the number of guests that will be attending for that one invitation.

If you are going to attend, I would squeeze in a definitely between the two words. If you are not going to attend, then put a not between the two words. Then on the side, put in brackets +1 or +2 or +3 to indicate the number of guests you are bringing. This is a weird way to word the invitation.

If it is addressed to a couple, fill in 'both'. If it is a single leave blank if attending. However, a line underneath in neat handwriting can be added 'I will be delighted to attend' or if you cannot, fill in 'not' . It is advisable to add to that one 'with apologies, but unfortunately I have long standing engagements for that date and will be out of the country'.

Maybe you put the number of people who will attend? Like will 2 attend. It is kind of bizarre.

You write "not" if you won't be attending. You put the number if people if you will be attending.

You print in "not" if you are not attending. If you are attending, leave it blank. If I were close to the people, or if it were family, I might squish in "happily" or "gladly" just to be cute.
