I saw a professional waiter do it recently and it was just beautiful. There was no wasted movement, and no struggling.

First of all, it pays to have a decent wine opener, perferably one that has a small blade to cut an opening slit on the foil located under the lip of the bottle. Run the blade around the lip, and lift the foil in a circular motion removing the all the foil from the neck to prevent cuts to your fingers by sharp foil edges. Next take the corkscrew at angle with the lever pointed out and locate center of cork. Give a slight push with the corkscrew into the cork to secure the point enough to prevent slippage while gently screwing the corkscrew downward. When you have screw in far enough till there is no longer any spiral, take the lever and place it on the top of the lip of the bottle, and pry upward until you can easily pull the cork out. DO NOT POP the cork! It is considered poor form in presentation. After the cork is removed, unwind the cork off the screw carefully avoiding covering it with your hands. Then present the cork to guest, or for just yourself, inspect the cork for signs of mold or wine seepage above the wine sealed end, or brittleness. It takes practice, but eventually it will come easy.

First it is important that the wine is stored on its' side so that the cork doesn't dry out. A dry cork crumbles and pieces get in the wine upon opening.
Usually you need to cut around the top to remove the foil covering the cork.
There are many kinds of corkscrews each having different ways of using them. Most of which will finish the job themselves (of course you have to do the work.) But the simple basic corkscrew you place the tip point in the center of the cork and screw in at least an inch or more.
Now to pull out the cork, keep the corkscrew firmly in one hand and with the other slowly turn the bottle while pulling.
I am sure you're waiter has had years of practice to perfect his performance. It is second nature after awhile.
And just for whatever it is worth... When opening champagne tilt the bottle to a 45 degree angle to prevent it from spilling out.

Push the cork completely into the Bottle, Then tie a Knot interior the tip of a sparkling Shoe lace, bypass the Knot surpassed the Bobbing Cork and easily pull, the cork will come out! get excitement from your Wine!

I've seen it to. First they use a foil cutter then a corkscrew and very carefully work out the cork.

Look, I'm still trying to figure out how to drink the stuff!!! Smell it, Swirl it, taste it, drink it. What the heck, lets just open it from the top and drink it.............
