A true hamburger has only beef. There are actually laws in the U.S. specifying only a beef product can be called a hamburger patty.

Hamburger, or ground meat, was first made popular in Hamburg, Germany when it was cooked in patties and served on a bun as an alternate to the Munich sausage on a bun. And you're right, there is no ham in hamburger. It wasn't known as hamburger in Germany, but when the dish was brought to America and became popular here, it was called the Hamburg sandwich. Eventually, the 'sandwich' part was dropped, because it wasn't quite a sandwich, and it was just known as a hamburger.
However, adding ketchup was an American touch!
