It is to do with mental conditioning. If, whilst your friend or your brother were growing up, someone in their household screamed or something if, say, a spider ran across the floor, the brain would associate that image with fear, and grow up afraid of spiders. Equally, if they had a bad experience with a dentist the same thing could occur.

I have never known a fear of spiders, but I surmise that it may in part arise from the fact that spiders are not only "creepy" but silent while creeping about. A person may fear the fact that they have no control of the situation when a spider is present: the spider's "will" seems to conquer their own.
Fear of denstists surely is transferred fear of intense pain. Furthermore you cannot SEE the source of the pain as you can with many other types. The pain is also INJURY (or perceived as injury) , for which there is compulsion to avoidance, rather than pain from an inherent condition. The mouth is sensitive, and though it may not really "feel" pain any more intensely, it will be recognised more readily.

Because some species of spiders such as black widows and brown recluse spiders are extremely poisonous. It's called being careful. As for being afraid of dentists it's more because getting anything done on your teeth is painful as hell. I'm not afraid of visits to the dentist but in the same token, visting the dentist doesn't rank too high up there on my list of things I look forward to.
