I am 66 years old. I would like to submit written request for Medicare, but I don't know
procedure.

you would have been eligible for medicare at age 65 and also you will be eligible for the medicare D prescription card.Go to www.socialsecurity.gov or go to your local social security office and sign up.Be sure to sign up for medicare part b also.There will be an 85.00 dollar premium deducted from your monthly social security check.Medicare is like any insurance plan they will only pay 80% on most things so look into a supplemental plan.My parents have an excellent plan with United American.What medicare doesn't pay they do.

Medicare is reserved almost entirely for hospital admissions, and when that occurs, the hospital contacts medicare for payment. If you need medical care and cannot afford it, medicaid is the program you need. Contact your local department of social and health services, they will guide you thru the process.
