When I was in Air Force JROTC in high school our instructor who was a Colonel told us about someone he knew who had been awarded the Congressional Medal of Honor and had gotten an Oak Leaf Cluster placed on it, signifying that they had been awarded the medal multiple times. Has this ever happened? If so was it a recent occurence? In what branch of service was the individual awarded? When I asked my step dad who was in the Air Force for 20 years about he told me that only two Airmen had ever been awarded it, once in Vietnam and once in the first Gulf War. Was just curious to clarify...

Nineteen men received a second award: 14 of these received two separate Medals for two separate actions, and five received both the Navy and the Army Medals of Honor for the same action.

I know for certain that there are two Marines that have been awarded the Naval Congressional Medal Honor on two separate occasions. GySgt Dan Daly in 1901 during the Boxer Rebellion in china and again in Haiti in 1915, and Maj. Smedley Butler in 1914 at Vera Cruz and Haiti in 1915.

These are the only two Marines that would wear star devices(Naval device for multiple award) on their Naval Medals of Honor, while those that have won the Army and Navy medal of Honor would wear two medals, as they are two separate awards. This would be similar to a prior service Army individual being awarded his having two bronze stars in the Army, then winning a third during the service in the Marines. He would wear the bronze star with oak leaf cluster, and a separate bronze star, to which would be added a star device on the ribbon.
