It means it could go either way and no body wants to tell you which way is most likely.
There are so many ways to violate a pts confidentiality its a nice way to really say nothing to inquiring minds and get by with it.

It means a condition that requires follow-up to see it may become worse!! It is a scale like this to determine the prognosis:
1) Bad (poor) 2) Guarded 3) Fair 4) Good 5) Excellent

guarded is the term used when a patient's vital systems are stabilized at any given time but anything could happen and they can become worse,hence the term guarded
