Say "hey, I like you... do you wanna go out sometime?"
If you are too embarrassed to do that, write her a note. That way you can say exactly what you want to and you dont have to be there when she reads it. Plus she can just write you back with her answer. Less embarrassing for the both of you and it gets the job done like a charm.

If you are sure that you like her, then ask her how she would feel about going out with you to do something that you know she would enjoy doing as a your date. That would allow her to express her level of comfort as far as the two of you dating. Make sure that both you guys really want it first because once you cross that line its hard to go back to being just friends - it can be done but nontheless its hard to do.
