They are both extremely awkward sentences. I think you did understand the problem of your sentence being whether or not your verb should be singular or plural, which is a good thing, and shows that you are thinking about your grammar.
The type of question you are asking is not one you would tend to ask conversationally, but more as a questionnaire type question.
I'm going to give you an alternative and see what you think of it.
Not "How many" but "what".
What is the most number of times you have washed your face in a single day?
I added "single" because that clarifies and specifies the nature of the time element of the question.
It's always better to keep the verb and subject as close together to possible (to avoid the confusion you faced in your sentences--to which noun does the verb attach itself, therefore, do I want the singular or the plural). I think my alternative solves those problems for you, and I hope helps you in your continued exploration of this wacky, wonderful language we call English.
