It doesn't have to be three, but it definitely has to be one other person...and the reason for this is because women want to know what the other girl thinks of the situation thats going on. So for example if you are double dating or something and both girls go to the bathroom, the first thing that they ask each other is what do they think about the guy? What do they think about the way he presents himself? The way he is dressed and etc. Once they have discussed what is happening outside...then depending on if the girls like the guy or don't they mentally devise a plan (individually) to either take things up a notch or not. By this I mean let's say that the girls are done talking about the guy then, if they agree that he seems like a nice guy or something then the girls will continue to enjoy the date and have fun...if they think the guy is a shady guy and cannot be trusted or seems like a loser for whatever reason then many girls change their behavior...some become mean, some try to end the date early and etc...
But to basically answer your question women go to the bathroom in pairs to assess the situation and figure out what to do next ...in addition to talk and gossip.

My guess is one to wipe, one to tickle, and one to scratch that hidden itch that girl has after she is done. Or otherwise they hold hands in the restroom and sing Kumbaya.

That's what you call buddy-buddy system. It is safer that way specially if you are in public places. Sort of a protection or security.
