A portfolio assessment is usually a box (or file) of samples of children's work. Every so often (pick a time frame- every two weeks, etc) take a sampling of the children's cutting, writing/drawing, and write on the back the child's name, date, & age of the child, as well as any notes you think are important (such as which hand they are writing with, how they are holding the utensil). At our preschool, we use pizza boxes to hold stuff (you can usually get them for free from a local pizza joint) and then, we can later jot the information into our assessment (such as the Desired Results Developmental Profile). We have also taken digital photos of the paintings, writings, etc or scanned them into the computer so that we can send the artwork home. We also keep a copy of their profile in the box so that we can add things as we think of them. I also carry around a note pad so that I can jot things down as I see them in the class, and while during circle time and outside time. Digital pictures are a great way to see a child's development and capture evidence of it. Take photos all the time during the day, and put them into the child's file. Hope this helps!

If it is an assessment for yourself to show your teaching, then you should model it like an elementary teaching plan. Write up the plan explaining the lesson and have either actual kids work and/or pictures of the kids working on the project. It is a good idea to explain the theme of the current unit and how this project relates and any additional things you did...books you read, songs, activities, etc. The more pictures and children's work in a portfolio, the better.
