I don't - you pay for the service and they are just doing their job. Having said that, if someone did an amazing job on a difficult set of alterations or did a very very last minute rush job, I might add a tip.

If you want. Tips should usually be given to people who can give you something for free, at a better rate, faster, etc, at least once in a while. If you're getting hems and alts. every week and you get a break, give a tip. If you're belly up to the bar every week and you get a free drink, tip a little more. The pizza is delivered in under 30, give a good tip...

Tip the tailor and 2 weeks later that hem comes undone???, you'll be paying for a new hem...

No. You pay for the jeans and the service is included. If you bought the jeans somewhere else, you would have had to pay for the alteration, so you would still not tip.

I'd think no. They probably get paid enough. I'd would like to know what kind of place you shop in that this service is even offered.
At 4' 11" I have to shorten every pair of pants I buy. Can you send me that info? Thanks
