It's not instant, but it is pretty fast. It's basically two computer modems talking to eachother over a phone line.
The sending fax uses a camera of sorts to scan the document to be sent, and encodes it digitally. The digital information is sent over the phone line to the receiveing fax, where the receiving fax basically prints it.

A fascimile? It is instant more or less. Once it scans your paper, it converts it to data (ones and zeros), and than calls the other fax over the phoneline. The other fax in turn takes the ones and zeros, and converts it back to an image, and prints it onto paper.
