It might mean to ask a woman whether she is ready 'to eat it raw' / to give a ********.

Or it could also mean that you've been having sex for so long that you've been rubbed raw.

Usually it means having sex without a condom. other terms like "raw doggin'" or "bare backin'" are common in that reference to sex without using a condom.
