It depends...There are tickets that have no refund (usually the economic ones, the ones that cost less) or have penalty charges for cancellation and there are some where you get full-refund (usually bussiness class tickets).
In any case, you have to ask for refund always before the date of travel written on the ticket, because the economy tickets usually are not valid after the date of travel, so no refund at all.

It depends upon what type ticket you bought and whether you bought it directly from an airline, rather than through a web site like Priceline.com. Some carriers will give you a credit or discount on your next flight for an unused ticket. When you buy a southwest airlines ticket their web site will ask if you have an unused ticket that you want to credit to the new purchase

it depends on what the fare rules on your ticket you bought are. If it is a non-refundable ticket, you will be given a voucher to use for up to one year. If you bought a refundable ticket, of course you can refund it. Some airlines do though give a penalty charge when changing the ticket including re-using the flight voucher.

That only depends on the Airline policy and how you bought the ticket. It it was on sale, most likely not. Anytime you buy a ticket, make sure you know the return policy for such cases.

It depends on the restrictions you agreed to when you bought it. Check the terms of the sale. There are restrictions codes on the ticket that can tell you more (usually one letter codes).
