Ф.__________ (its a russian f)
И.__________ (its a russian i)

F: Family name, for example Petkov.
I: First name, for example Dmitry
O: Middle name, for example Nikolayevich.

Middle name is father's first name followed by -evich or -evna. So in this case, Dmitry Nikolayevich Petkov's dad's name is Nikolay, and because he's male his middle name is Nikolayevich. If it's a girl, it would be, for example, Olga Nikolayevna Petkova (some Russian women's last name ends with -a, but in America many just keep the masculine as the last name).

If you wanted to do it it would be, for example:

Ф: Smith

И: John

О: James

Familiya (last name)
Imya (first name)
Otchestvo (patronymic)

A "patronymic" is a special name Russians use as their middle name. It is their fathers name with a special suffix. If you are male, you add the suffix "-ovich" or "-evich" to your father's name (depending on the last letter of the name), and that is your patronymic. If you are female, you add "ovna" or "-evna". For example, Mikhail Gorbachev's father's name was Sergey, so his full name would be Mikhail Sergeyevich Gorbachev. Or, if you wrote his name on your dog tag, it would look like this:

F: Gorbachev
I: Mikhail
O: Sergeyevich

the N stands for "imia"-name
the O stands for something that means middle name (which in Russian is just your father's name)
