I know about the limit screws that control how far in and out it can swing to the inner and outer chainrings. Those seem to be fine, but with the chain on the MIDDLE chainring, it drags against the inside of the derailleur. How do I fix that?

The front shifter is indexed. It's a Shimano grip shifter from about 1993.

sometimes when that happens (even with just 2 chainrings) I just lightly tweak the shifter, one way or another, and that moves the cable enough to get the derailleur cage centered properly.

Unless your front is indexed. I think most are not though.
