If a person was in Iraq, they have relative that are US citizens. That person apply to get a visa. But due to increased military anctions and sectariam violence they can not stay where they were and flee to another nation, where they had no time to get a entry visa or visa of any kind. Could the existing visa app be transfered to the enbassy in the nation of refuge, and by fleeing to there with out a visa hamper the visa app to the US. Or would you have to start from scratch once there(the nation of refuge)? Or if they got refugee status would that make a differance? And could the embassy in the nation of refuge get the started application transfered in?

Visa is only a privilege. Once it expires it is no longer valid.
Another application for visa should be filed in the country of refuge.

If you are talking about an immigrant visa, it can be transferred to another US embassy or consulate anywhere in the world to be processed. Your petitioner should call the Service Center to find out how to request that: it's simple. We do it all the time.

US consular officers don't care if you are in another country illegally; isn't it clear that just enforcing our own immigration laws is as much as we can handle?
