It depends on the kind of relationship you had with the person, why you havent spoken or seen eachother in so long, and what you are hoping to accomplish by writing them. Your question is too vague, and because of such, I'd take what other people say with a grain of salt. Something like this is circumstancial.

Be honest and sincere. After all, if you haven't seen them, they haven't seen you, so it's a two way street. Communication, works that way.

A formal letter is Dear..... but you could say "hey (name) we didn't talk in a while. i hope you still remember me we should get together sometime" add more info if you want
