It is a term given to the painfrul ache in the groin a man gets after being sexually aroused for a long period of time without climax. It absolutely does exsist, with the exception that the tesicles do not actually turn blue.

You do not physically get blue balls, but it is a term when you are sexually aroused for a long time and do not get orgasmic release. The blood pooling causes cramping in the region and thus the term.

It's supposedly a myth where a guy gets an erection but a girl won't have intercourse with him so it makes the testicles hurt.
Personally I think it's a way to make women feel bad for you when you're young.
