find out what board certifies it, and retake the exam.

It first depends on the state in which it was held. Go to your state's ".org" website and follow the links to the state board, licensing, and so-on. You should find your answer there. Good luck.

You should just re-apply and pay the necessary late fees. Just ch with your state board and they will let you know. It should be easy if it hasn't been years............ Good luck
