If you max out once, you can tear the muscle in a way that it won't grow back stronger. when you lift weights, you tear your muscles. they grow just a little bigger to compinsate the weight you lifted befor. That way you can lift even more later.

If you are talking about weight training, sets are much more effective. They also give you a goal to work towards.

Main reason for sets is to "train" your muscles. If you don't exercise your various muscles consistently and repetatively, you can have problems like cramps, imbalance and fatigue.
