Many insurances that are nationwide (Aetna, United Healthcare, Blue Cross.Blue Shield, etc.) do not cover the surgery. Sometimes it's by the choice of the employers what riders get attached to the plans, but many don't cover it because it's not what they deem medically necessary.

Have you asked your doctor to give proof of medical necessity for the surgery to your current plan? Sometimes, if it can be proven beyond a reasonable doubt that a procedure is necessary to save the life of the patient, some insurances reconsider. They may not cover it in full, and they always have the option of not covering it at all, but it's worth trying.

Good luck.
