The first step would be to find the phone number of your nearest Catholic parish, call them, and set up an appointment with a priest (or maybe a deacon) to speak about converting to Catholicism.
The parish itself will have an RCIA program, or if it doesn't, will guide you to a nearby parish that does.
RCIA stands for Rite of Christian Initiation for Adults. I'm not sure how long RCIA classes last, but I believe they are less than a year.

You sign a statement renouncing any other religions, and are baptized again. You must have education/counseling sessions with a priestbefore this, and it would depend on him how long it takes. In some churches there would be classes. You will need to be confirmed the first time the bishop comes to visit.

Yes.
It involves ritual, including baptism. If your married you will have to be remarried.
Depending on the church or diocese you may have to obtain some rites such as holy communion, reconciliation and confirmation in that order.
I am not sure if the process is standardized by the Vatican or not.

Yes there is a formal process, catechism classes and the sacraments, it could take awhile, contact a priest at any parish and they can tell you the specifics.

Yes there is, but it all depends on what you are converting from? It is less involved if you are coverting from another rite of Catholicism (i.e. Byzantine, Marinite) as opposed to converting from Presbyterian.

It takes one or two years.
You attend classes to learn about the religion, and you go through a few rituals. At the end, on Easter Vigil, you get Confirmed (i.e. you get oil smeared on your head), and if you havenpt been baptised, you get baptised (water sprinked on your head).
You then get a certificate with your chosen saint's name on it, and from then on, you get to take communion with the rest of the Catholics.
